Report a Claim

All claims must be submitted with completed claim forms and proof of ownership. Proof of ownership can be submitted in the form of purchase receipts, a copy of the title page of the user's manual (which displays make and model), or a copy of the box with UPC.

All iPhone and cell phone claimaints must submit a copy of the receipt of purchase for the new phone in addition to proof of ownership for the original phone.

DAMAGE CLAIMS ONLY! DO NOT dispose of your damaged item. You may submit the damaged item with your claim forms if no other proof of ownership is found OR you may be requested to remit your damaged item after payment of the claim has been made.
