All iPhone and cell phone claimants must submit a copy of their driver's license, a copy of their phone upgrade status, in addition to proof of ownership for the original phone.

Damage Claims Only!

DO NOT dispose of your damaged item. You may submit the damaged item with your claim forms if no other proof of ownership is found OR you may be requested to remit your damaged item after payment of the claim has been made.

Caution

Any person who, with intent to defraud or knowing that he/she is facilitating a fraud against an insurer, submits an application or files a claim containing a false or deceptive statement is guilty of insurance fraud, a Federal Offense.
