Step Two: Select the policy for your damaged device from the list displayed at the bottom of the page under “Your Policies.”
Step Three: On the “View Policies” page, scroll to the bottom of the page and click the blue “Submit Claim” button
Step Four: Follow the instructions on the page to enter the details of your claim, then click the button to submit.
